Meritan Brings Community Aging in Place—Advancing Better Living for Elders (CAPABLE) to Shelby County.

A national program to help seniors age in place is available in Shelby County thanks to Meritan’s Home Health healthcare professionals called “Community Aging in Place—Advancing Better Living for Elders (CAPABLE).” CAPABLE is a low-cost program developed at the Johns Hopkins School of Nursing for low-income seniors to safely age in place.

The approach teams a nurse, an occupational therapist, and a handy worker to address both the home environment and uses the strengths of the older adults themselves to improve safety and independence. Meritan's healthcare professionals are CAPABLE trained, and we want seniors to have the choice to live at home or with family and stay safe.

Carrie Herron Nemeth, one of Meritan’s Occupational Therapists, stated, "“Part of Meritan’s mission is to improve quality of life and well-being for our patients. In Home Health, we are working with patients who, oftentimes, have lost their independence and are somewhat at the mercy of the healthcare system. CAPABLE emphasizes the patient being an active participant in driving their outcomes through therapy which, in turn, focuses on more meaningful outcome measures being met. Being certified through CAPABLE is another tool in our kit to allow us as clinicians to focus on comprehensive patient-centered care.”
